Mac OS Security Update Failed

I have a PowerPC G5 running OS X 10.4.11 (running client version 7.2.6.4) that I am trying to update with some of the fixlets from the Patches for Mac OS X site. Specifically, I have tried to run “Security Update 2008-008 (10.4.11 Client) - PPC” and “Security Update 2009-005 (10.4.11 Client) - PPC”. After the fixlet runs, the status shows “Pending Restart” and after the user reboots, it changes to “Failed.” When I look at the Action Info all steps show completed. The workstation continues to be relevant for these fixlets, so it appears as though they are indeed failing, but how can I find out where the issue is?
