I’ve obtained an Internal Use Only license and am trying to install a
production server (I previously had an eval server running on same
machine). I am getting the subject error when trying to generate the
masthead file. Any ideas on what the issue may be?

I will close the loop on this problem. There apparently were artifacts left over from my eval install. I was directed to the TEM Wiki:
to download and run the “TEM Remove Utility”. After this utility cleaned up remaining eval installation, I created another license file and was able to successfully install a production server.
