Many times I see, when ever I apply any action it fails in some of the clients.
Here I have applied the action and it failed, in action info it has completed all the steps and show completed. I could not find any difference with the action info of the client where same action was successful.

When you see an action that failed like this it means that the action ran, but the underlying condition it was trying to fix was not fixed. In you particular case, you ran the patch successfully, but it didn’t actually patch the computer. This sometimes happens with patches (no matter how you run them). Many times, restarting the computer and re-running the patch will work.
